Abstract
X and P control charts are used to monitor human error with Material Control and Accounting data at Nuclear Fuel Services, Inc. Charts use data from existing systems which include process monitoring data, Material Control Reports, computer transaction data, and item monitoring data. The X-chart is used to display input-output differences at material control units, and the P-chart is used to display the proportion of errors per batch, transaction, or item monitored. Computer databases are built to provide data for long term error estimates and for queries on system performance. IMPORTANCE: This is an attempt to address some of the requirements of 10 CFR, paragraph 74.59, that specifies incorporating \"checks and balances in the MC&A system sufficient to control the rate of human errors in material control and accounting information.\"
